North Somerset LD

Within our services, we have a small team of professionals in North Somerset, including:   

• administrative staff  
• support workers  
• learning disability nurses    
• psychologists   
• psychiatrists 

Our teams work alongside other  professionals:  

• occupational therapists  
• creative arts therapies services  
• social workers  
• GPs and paediatricians   
• specialist schools for children with learning disabilities  

We are a specialist service supporting children and young people: 

• from the age of 0 to 18 years (or up to 19 if in full-time specialist education).  
• with a GP registered in North Somerset. 
• with a moderate, severe or profound learning disability with complex needs, including emotional wellbeing and mental health issues. Evidence of this learning disability will be shown in relevant reports, such as educational psychology reports and the school Education and Health Care Plan (EHCP).
• who have been referred from multi-agency professionals who have worked with your young person, for example school SENDCo and teaching staff, social workers, GPs, paediatricians and hospital clinicians. 
• who have already accessed some relevant community level support. 
• we have received consent from them or someone else on their behalf to provide support. This consent needs to be written on the referral form.

I have a mild Learning Disability. Where can I get support?

If you are concerned that you, a child or young person you care for has mild learning disabilities, autism or other specific learning difficulties, there is mental health support available. Please visit our conditions page for some advice about the support you might need. 

We cannot accept self-referrals, which means you are unable to refer directly to our services yourself and someone else will need to refer you to our service. People who can refer you include staff at your school, social workers, a family doctor (GP), a paediatrician and staff at a hospital. 

Will you ask for my permission or consent?

The person who refers you will ask for your consent. If you are too young or unable to consent, then your parents, family or carer can give consent on your behalf.

You can also access support from community paediatricians, health visitors, school health nurses, children’s speech and language therapists and children’s occupational therapists

We can be flexible to offer you and your family appointments at school, at home, sometimes in clinic, via telephone, via video conferencing, or a combination of options.
