Avon and Wiltshire Mental Health Partnership NHS Trust wants to make sure people have the best possible experience of care.
The Friends and Family Test Survey is a quick and easy way of giving views about the care recently received, whether that's from the service user, patient, carer, family or friend. This feedback helps us improve our mental health services. On 1 November 2024, we changed to a new provider: iWantGreatCare.
You will be asked a question about your overall experience of care, and 5 more questions so you can tell us how it was for you in more detail.
You can also leave comments for teams to see and act on. This helps us to improve how we deliver care for you and others.
The survey is offered at care review, transfer, discharge and whenever you wish to give your views. The survey is confidential and any comments will not be traced back to individuals. If you are taking part, please tell us exactly what you think, whether it is positive or negative.
