FIND is the Forensic Intellectual, Neuropsychiatric and Development Disorders team with an outreach and liaison pathway.
FIND works with people from Somerset, Bristol, North Somerset, Gloucestershire, South Gloucestershire, Bath, Swindon and Wiltshire. We complete gatekeeping assessments for people with learning disabilities, brain injuries, or autism who present a risk to others and may need admission to secure services.
Our team provides an outreach and liaison service to support individuals leaving hospital to return to the community. This support starts 12 months before leaving hospital and continues until 12 months after discharge.
We also provide consultation to health and social care commissioners, community teams, non-secure hospitals and providers working with people with learning disabilities, brain injuries or autism where there is significant concern of risk to others.
Our secure services are members of the Quality Network for Forensic Mental Health Services (QNFMHS), a quality improvement network for low and medium secure forensic mental health services across the UK.
All referrals to secure services should be made via an online portal SWPC Patient Flow System. For log in details and a guide on how to use the system, please email dpt
