The Care Home Liaison service provides mental health assessment and interventions to support care home residents. The service supports, educates and trains care home staff in managing and caring for older adults with mental health problems.
Our aim is to reduce distress among residents and promote their wellbeing by helping staff in care homes focus on an individual’s needs. We also aim to minimise the need for admission to hospital. If admission is needed, we try to make this as short and as smooth as possible.
The Dementia Enhanced Support Team (DEST) is an intensive service that works closely with Care Home Liaison teams. Both teams work with older adults in the acute phase of an illness and experiencing a mental health crisis. Care Home Liaison works with older adults who have organic or functional illnesses who live in a care home, and DEST works with older adults who have an organic illness living in their own home or in a care home.
DEST and Care Home Liaison take referrals from GPs via the Primary Care Liaison Service, care home providers, community care and housing, Sirona, and any other services within AWP.
