Shared Care Records are a local electronic patient records that allows health and social care professionals directly involved in your care secure access to a summary of your medical record.
Your Shared Care Record will help those caring for you to manage your care better by allowing information to be shared quickly and safely. Only authorised staff providing health and social care services in your area can access your record.
The four systems the Trust uses are:
1. Connecting Care system, which covers health services across Bristol, South Gloucestershire and North Somerset (BNSSG)
2. The Bath, Swindon and Wiltshire (BSW) ICR, which covers services in Bath and North East Somerset (B&NES), Swindon and Wiltshire.
3. The Somerset Integrated Digital e-Record (SIDER+) which covers Somerset
4. The Devon and Cornwall Care Record (DCCR) which covers Devon and Cornwall
Avon and Wiltshire Mental Health Partnership NHS Trust uses these systems so we can access your data stored within the system and to provide relevant information about you and your health to others involved in your care.
